Information collected from any user of the HealthChoice website shall be used or disclosed for official purposes only, or to comply with any state or federal law. HealthChoice will not intentionally disclose any Personally Identifiable Information (PII) of its web users to any third- party, unless otherwise required by law. 

HealthChoice recognizes and respects individuals’ right to web or online privacy, including individuals’ right to privacy under certain state and federal law. HealthChoice will adhere to the mandates of such laws as: HIPAA, Michigan Internet Privacy Protection Act, Social Security Number Privacy Act, etc. Alternatively, HealthChoice also recognizes and respects all state and federal law, which may require HealthChoice to disclose certain information. They include requests made under the Michigan Freedom of Information Act (FOIA), subpoenas, court orders, or any other state or federal law, including for any law enforcement purposes. 

Information We Collect 

When you use our website we may collect information that identifies you, as well as online data about you.
“Personal Information” – Certain Personally identifiable information (PII) may be collected from you if you voluntarily provide the information in order to make a premium payment online. 

Any information submitted online for this activity will be treated as if you had engaged in this activity in person rather than over the internet. The type of information we collect may include your name, mailing address, email address, telephone or wireless phone number, social security number, or other information that identifies you. Our online services may ask for credit card or third-party bank account information to complete a premium payment transaction. After you log in to our online services, we associate subsequent activity during the session to you. 

“Online data” – Certain information may be collected from you automatically when you view our web pages. This may include: data such as your IP address, browser type and language, operating system, and the date and time you accessed the HealthChoice site. 

How We Use Information We Collect 

  • To enable you to use online tools or perform certain online transactions
  • To communicate with you about your account and the services we provide to you
  • For security purposes. We may use your information to verify your identity and/or location to allow access to your account, conduct online transactions and assist us in taking data security precautions and to engage in fraud prevention/detection

Who We Share Information With and Why

We do not sell, trade, or otherwise transfer to outside parties your personally identifiable information. This does not include trusted and contracted third parties who assist us in operating our website, conducting our business, or for the express purpose of delivering the service requested by the customer, so long as those parties agree to keep this information confidential.
We may share information to respond to a court order or subpoena. We may share it if a government agency or investigatory body requests. We may share information when we are investigating potential fraud. 

Opting out of Information Sharing

HealthChoice does not plan to share your information except as allowed by law. You are unable to limit sharing of your information since your information is only shared with those third parties contracted with HealthChoice to deliver the services we provide to you.
