Are you selecting a health plan for you or your family and need assistance finding a local provider? You have come to the right place! The NYS Provider & Health Plan Look-Up gives you all the information you need in one place! All you need is internet access. And…no personal information is required to use the Look-Up. Here’s how it works. Let’s say you already have a doctor or other health care provider. The Look-Up is an easy way to find out which health plans they participate with. Just enter the name of your health care provider or the name of their practice. Next, click SEARCH and you will see a list of health plans your provider participates in! If you don’t have a provider yet, no problem. The Look-Up can help you with that too. Simply select the health plan and the type of insurance you have and you will see a list of all the health care providers that participate with that plan. Looking to narrow your search? You can search by provider specialty, such as pediatrician, cardiologist or dentist.

Looking to refine your search further? Select your preference for the provider’s gender…spoken language…or wheelchair access. Once you find your preferred provider, the Look-up conveniently displays their address, phone number, and with just one more click, you have directions!
